WHAT ARE SUPPORTED LIVING SERVICES?
Supported Living Services (SLS) help individuals live more independently while receiving personalized assistance with everyday needs. Support may include daily living activities, personal care, household tasks, community participation, and other services based on each person's individual needs and goals.
At For A Better Future SLS, we focus on promoting independence, confidence, personal choice, and meaningful participation in the community. Our support is tailored to each individual and can be provided for as long and as often as needed to help them live a safe, fulfilling, and independent life.
WHAT SERVICES ARE PROVIDED?
At For A Better Future SLS, our services are personalized to meet each individual's unique needs, abilities, preferences, and goals. Support is continuously adapted based on each client's physical and emotional needs, as well as their short- and long-term goals.
Our team can provide assistance with daily living activities, personal care and hygiene, meal preparation, errands, medication reminders, household tasks, healthcare coordination, and community participation. When appropriate, additional behavioral and emotional support may also be provided.
Our goal is to provide the right level of support while encouraging independence, confidence, personal choice, and a greater quality of life.
IS THERE A REFERRAL PROCESS?
Yes. For A Better Future SLS works with California’s developmental services system, and individuals are referred to us through the North Bay Regional Center (NBRC).
If you are interested in receiving services from For A Better Future SLS, please contact NBRC and let them know that you would like to work with our team.
Once you connect with NBRC, they can provide information about eligibility and guide you through the referral process.
